Well, if we have to spend this much time trying to figure out what the FAQ says...that's one problem.
The second problem is that after the linked PDF was reported, Abuse also decided to challenge all of Katharine's fic...but here again, they were not clear on what basis. Were they challenging her as the author? Challenging the account as being owned by her? Both? No clue given as to what proof she needed to provide. So we threw everything we could think at them and hope it was sufficient. And one of their follow-on requirements made little sense. "Link to the original fic if possible". (a) A link requirement for any fic (zine/online) is not spelled out anywhere. (b) The zine stories *were* credited as zine stories with titles and dates etc. If they are requiring a Fanlore link because they were zine stories they should also spell that out.
And last...and this is the biggest problem...there is no review process before they permanently and irrevocably delete your account or your fanworks. It is a decision that is made by only 2 people (although I suspect the might ask around informally) and there is no undo button. So if there is one change that comes out of this, is that AO3 needs to implement two stage deletion and a clear escalation path. Hopefully someone with more weight and insight than me will take take this up through whatever channels they have in place today. I submitted a Support request for 2 stage deletion with a link to this entire post.
